Important: June 2016 Wellness Discount
ALL TEAM MEMBERS AND SPOUSES (IF APPLICABLE) MUST RE-QUALIFY FOR THE WELLNESS DISCOUNT FOR JUNE 2016.
If you and/or your spouse previously qualified for the wellness discount, you must re-qualify in order to continue receiving it for the June 01, 2016 Plan Year. Once qualified, you will be eligible until May 31, 2017.
The deadline to complete the steps and receive the full annual discount is May 13, 2016.
Team Member and/or spouse who met all of the wellness discounts requirements after March 01, 2016. They may continue receiving the wellness discount until May 31, 2017.
How much is the wellness discount?
The wellness discount is an annual amount of $800 or $15.39 per week per person. Both the Team Member and spouse (if applicable) are eligible to receive the discount. Note: The spouse must be enrolled in the medical plan to receive the wellness discount.
Can I receive my wellness discount at anytime?
Yes. All eligible participants may complete the necessary steps to qualify for the wellness discount at anytime and begin receiving the weekly discounted premium from that date forward, until the end of the Plan Year.
What steps are necessary to qualify for the wellness discount?
- Make an appointment with your doctor to complete the Health Qualification Form (HQF). A link to the form is below.
- Tell your doctor's office you will need lab work for your cholesterol and blood sugar, which you may be able to do before your appointment.
- Send the completed HQF form to BC Idaho. Submission instructions are on the second page of the form.
- Take the online Personal Health Assessment (PHA) at members.bcidaho.com. (A spouse must register under their own log in.)
You need 65 points on your HQF an must take the online personal health assessment to qualify for the medical premium discount. Keep in mind, you can take action at any time, but completing these steps by May 13 means you'll pay the lower premium rate for the entire 2016/2017 Plan Yea.r.
If it is unreasonably difficult for you to meet the program standards or medically inadvisable to do so, you can complete a waiver form with your doctor, and submit it to BC Idaho instead of filling out the HQF. Contact the BC Idaho Customer Service Department at 1-866-283-6808.
