Important 2015 Tax Information: New Tax Form 1095
The 2010 Affordable Care Act (ACA) requires that all Americans have health care coverage either through their employer or the government marketplace.
Beginning in 2016, your employer must provide you and the Internal Revenue Service with a new tax form detailing your 2015 heath care coverage: Form 1095. This is an important form that you may need to complete your 2015 tax return.
What is Form 1095?
The form is required by federal law as part of the 2010 Affordable Care Act. The form reports:
• Whether your employer offered health coverage in 2015.
• If the coverage was affordable according to the ACA standards.
• Details about the cost of coverage offered you.
Are there different versions of the form?
Depending on the type of coverage you had in 2015, you will receive one of three different versions: 1095-A, 1095-B, and 1095-C. Most Albertsons Companies team members will receive Form 1095-C from the Company. Additionally, some team members who are enrolled in health coverage under a union-sponsored plan, will also receive a 1095-B from the coverage provider.
Who will receive a 1095?
Full-time team members (averaging 30 hours or more per week) and part-time team members (averaging 30 hours or more per week) enrolled in health coverage through your employer at any time during 2015, will receive a 1095-C. If you were employed by more than one company in
2015, you may receive a 1095 from each employer.
When will I receive my 1095?
The Company plans to mail Form 1095-C to team members' homes in early 2016, as soon as administratively possible, via regular postal service.*
Please ensure your home address is updated in the system to prevent any delay in delivery.
*Per IRS rules, the company has until March 31, 2016 to mail 1095 forms.
What should I do with my Form 1095-C?
You may not need your 1095-C to file your 2015 taxes; however you may need the form as a reference. Please retain your 1095-C form for your personal records.
